Why not download the Health Passport, an app for your phone packed with updated information on activities for young people, meaning there is no excuse to be bored.

It is also an easy way to share painful and personal information, such as a history of abuse or medical matters, without talking about it or repeating it.

To download the app: Go to App Store on the phone and search for the My Health Passport APP and download it
or
you can access via the QR code on the flyer which does need to be down loaded.
or
If you have an iPhone and have the App already on your phone you can scan to down load Health Passport APP

Once downloaded click on the three lines on top right of the screen and look at option my details. This is where your information can be stored.
Young people can download the “My Health Passport” app on their phone. The APP provides information such as:-
